Eligible State Health Department Applicants: State health departments from any of the 50 US states and the District of Columbia are eligible to apply, with the exception of Arkansas, Colorado, Florida, Louisiana, Idaho, Indiana, Iowa, Maine, Massachusetts, Michigan, Minnesota, Mississippi, Montana, Nebraska, New Hampshire, New York, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Texas, Utah, and Wisconsin. These states are excluded because they have already participated in the GIS Training for Surveillance of Heart Disease, Stroke and Other Chronic Diseases.

Key dates for the RFA for State Health Departments:

  • State Applications are due January 10, 2014; notification of award by February 14, 2014.

Eligible Local Health Departments Applicants: Eligible local health departments (LHDs) include all LHDs located in states in which staff from the State Health Department have already participated in the GIS Training for Surveillance of Heart Disease, Stroke and other Chronic Diseases. This includes LHDs in the following states: Arkansas, Colorado, Florida, Louisiana, Idaho, Indiana, Iowa, Maine, Massachusetts, Michigan, Minnesota, Mississippi, Montana, Nebraska, New Hampshire, New York, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, Texas, Utah, and Wisconsin. LHDs that have already participated in this training are not eligible to re-apply.

Local Health Department RFA Key dates:

  • Local Applications are due January 17, 2014; notification of award by February 21, 2014.
