Fireside Chat: Advancing Team-Based Care Through the Use of Collaborative Practice Agreements and Using the Pharmacists’ Patient Care Process to Manage High Blood Pressure

NACDD and CDC are hosting the next fireside chat focused on advancing team-based care, specifically on 1) the use of the pharmacists’ patient care process and collaborative practice agreements, and 2) the experiences of states participating in a workshop-based project designed to implement new resources from CDC. Fireside Chats provide an interactive format for state health agencies to hear from content experts on a number of topics pertaining to cardiovascular health and 1305/1422 priority areas.

Register now for the October GIS Virtual Training for Chronic Disease Prevention


Beginning this fall, CDC, NACDD, and Rice University will offer a series of trainings on a range of GIS skills to support the work of state and local health department staff for chronic disease surveillance and prevention. Each of the trainings will take place during business hours for one week and will include a combination of webinars, assignments and office hours, with an expected participation commitment of 10 hours. The first training, Mapping the National Provider and Plan Enumeration System (NPPES), will take place Tuesday, October 10 through Tuesday, October 17. Participants can enroll on a first-come, first-served basis until all 25 spaces are filled. Click here for details on the training series and to register.

CDC Health Systems Scorecard Tool


The Health Systems Scorecard (HSSC) is a new quality improvement tool to assess evidence-based chronic disease care management policies and practices. CDC encourages state and local health departments to share this assessment tool with their health system partners. CDC’s hope is that health systems can use the HSSC to assess the policies and protocols they are using to treat patients with certain chronic conditions, and that this quality improvement tool will help stimulate clinical-public health partnerships at local and state levels. The HSSC Tool is available at here.

In July, 2017 NACDD hosted a webinar to promote the use of the HSSC. Presenters provided an orientation and demonstration of the electronic tool and its primary components, as well as described how to set up an account and invite health systems to participate.
