The National Association of Chronic Disease Directors (NACDD) is a public health association for chronic disease program directors of each state and U.S. territory. Since its founding in 1988, NACDD has been a national leader in the effort to reduce chronic diseases by mobilizing its members to advocate for preventive policies and programs, encourage knowledge sharing and develop model partnerships for health promotion through state and community-based prevention strategies.

The CVH Council is comprised of program managers and staff of heart disease and stroke prevention programs in state health departments around the country, as well as partnering agencies that support the work of heart disease and stroke prevention programs. The Council provides a forum for managers and their staff to come together to share their wisdom, experience, successes, and lessons learned in an effort to make their work more efficient and more effective. The Council is also committed to developing necessary training, communication, and partnerships to enable program managers to better meet the challenges of their job. The Council has set an ambitious agenda for 2009, and we need the participation of every state to get the work done. Some of the tasks we have set for ourselves include:

  • Increase the Impact of State-Based Programs
  • Work with Key Partners to Shape the National CVH Agenda
  • Establish and Promote the Council's Unique Role and Identity
  • Expand and Diversify CVH Funding
  • Continue to Build Council Capacity

We hope that state CVH program managers and staff will use this site to learn about the Council’s activities and join in. The strength of the Council is the membership, so the more the programs are willing to share resources, success stories, tips, ideas, tools and other useful information, the better the Council will be able to fulfill its purpose.
