Recommendations for Integration of Chronic Disease Programs: Are Your Programs Linked?
- Comprehensive and Integrated Chronic Disease Prevention Action Planning Handbook — Developed by Partnership for Prevention, the handbook provides guidance and tools for states and communities to use in developing and implementing integrated chronic disease prevention programs.
- Integratng Chronic Disease and Maternal Child Health Programs - With more emphasis on the importance of chronic disease prevention and management for women of child bearing age, especially related to obesity, diabetes, hypertension, and smoking, integration of chronic disease and maternal child health programs is becoming an emerging issue. By using chronic disease related data from the Pregnancy Risk Assessment Monitoring System (PRAMS), state programs can identify ways to work together to improve chronic disease prevention and health promotion efforts targeting women of child bearing age. For more information, see PRAMS: What's In It for Chronic Disease Programs?
