The National Association of Chronic Disease Directors (NACDD) is a public health association for chronic disease program directors of each state and U.S. territory. Since its founding in 1988, NACDD has been a national leader in the effort to reduce chronic diseases by mobilizing its members to advocate for preventive policies and programs, encourage knowledge sharing and develop model partnerships for health promotion through state and community-based prevention strategies.

Welcome to the National Association of Chronic Disease Directors’ Diabetes Council!  On behalf of the membership, thank you for your interest in the Council. The Diabetes Council is a resource for state- and territorial-based Diabetes Prevention and Control Program (DPCP) staff for building infrastructure, learning best practices, collecting data and evaluating the effectiveness of their activities. This member organization works to serve you!

The Diabetes Council voting membership is open to the DPCP Director/Coordinator (or his/her designee) from each state, territory, district, or commonwealth.  However, all DPCP staff is welcomed and encouraged to serve as non-voting members. Associate non-voting membership is open to partner organizations, NACDD associate members, and other state and territorial chronic disease program managers and staff.

The Diabetes Council’s Mission is to "define, prioritize, and address national public health diabetes issues and advocate for diabetes policy to support state and territorial DPCPs and other partners in order to reduce the burden of diabetes." To realize our mission, the Council has established six priority areas as follows:
