For the last ten years I have been working at Kaiser Permanente Colorado, most recently as Senior Manager for Disease Management and Quality Support Team which includes over sight of regional initiatives and population management for Diabetes, Depression, Asthma and COPD, Hypertension and geriatric health assessments and prevention care.
