Summary: Accomplishments

April 2004 – Present (11 years 6 months)
More than 30 years’ experience in various pharmacy and health care management settings including: Population Care Management, Programs for Diabetes and Heart Failure Transitional Care, Community Diabetes Prevention, and Management Programs. Chronic disease care management: use of multidisciplinary care teams to provide patient centered and coordinated care. Specializes in interventions that improve outcomes for chronic diseases. Key facilitator in the development of new programs: including Hospital Post-discharge Program, Heart Failure and Diabetes Transitional Care Programs, Management of Steroid Induced Hyperglycemia in Ambulatory Care Setting. Partnerships across disciplines to improve outcomes for diabetes. Conducts clinical practice research: interventions to improve chronic care delivery. Developed a pharmacist intervention: R-E-A-D. Designed to improve patient centered care, and address specific medication related problems. Is program director for community (Type 2) diabetes prevention programs utilizing CDC curriculum for lifestyle change. President to International Pre-Diabetes Center Inc. Is recognized for her ability to contribute conceptual leadership skills in the development of hospital quality metrics enabling the delivery of better patient care. Serves on Technical Expert Panels to Centers for Medicare and Medicaid (CMS) on Hospital Quality: for the design and development of electronic quality metrics and electronic-quality CARE tool, Hypertension and Coronary Artery Disease Quality Measures.
