• Coalition building coupled with enhanced service delivery – includes development of diabetes self management programs in underserved rural areas
• Diabetes Care Improvement initiative with documented improvements in health outcomes and measures
• Advocating for desirable and evidenced based care, services and support for the uninsured, underinsured and people with diabetes
• New project development and oversight, including implementation and expansion of the Stanford Chronic Disease Self management Program across a large rural region (the Upper Peninsula of Michigan)
