NACDD is a proud Million Hearts partner organization. One of our most exciting projects is working with state health departments to convene a one-day Million Hearts Stakeholders Workshop. These workshops bring partners together to focus on one or two aspects of Million Hearts related issues, to scan the landscape of current activities in the state, to learn about successful approaches others have used, to identify assets and opportunities, and to develop a set of strategies and priorities and the beginning of an action plan. With support from CDC and the states, NACDD has worked with 16 state public health agencies to plan and facilitate Million Hearts Stakeholders Workshops and to provide follow up technical assistance since 2012. Four of the states have now submitted success stories to NACDD's What's Working Database. Find out about the Iowa Million Hearts Action Plan, how New Mexico is working with faith-based organizations, tribes and pueblos, African American and Hispanic communities; how Michigan is increasing adoption of Electronic Health Records (EHRs); and how Texas is supporting the use of Community Health Workers.
