2015 Award Nomination Submission

Joseph W. Cullen Award for Outstanding Contributions to Chronic Disease Prevention and Control is given to an individual outside the traditional public health field who has made outstanding contributions in the field of chronic disease.
The Award for Excellence in the Prevention and Control of Chronic Disease is given for distinguished service and the "advancement of knowledge and practice in reducing the chronic disease burden." It is presented to an individual in the field of public health.

The Community Impact Award is given to an individual or team that has made a significant community impact or achieved significant influence in a community related to chronic disease control or prevention. Nominations can also include meritorious work involving coalition building and legislative advocacy efforts

Describe the activities and level of effort of the individual or group, which merit the award for which they are being nominated. If an individual is being nominated, describe the position they held while performing this work and any relevant publications. (500 character limit)

1) Only one person may be recognized per accomplishment, only one award nomination category per program or person will be considered.
2) Form letters are discouraged.
3) State chronic disease departments are encouraged to submit nominations as a unit and with the approval of the Chronic Disease Director.
