Coordinated Chronic Disease
Core Functions Assessment
In early 2013, a workgroup with members from CDC,
NACDD, and state chronic disease program staff came together to draft a survey
comprised of core evaluation questions designed to describe the impact of a
coordinated approach on chronic disease prevention and health promotion at the
state level. The workgroup
reviewed chronic disease capacity frameworks, public health infrastructure
assessment literature, and relevant FOA guidances to develop the resulting tool
that will be used to collect a uniform set of data that will enable the us to
all understand how chronic disease prevention and health promotion practice is
evolving through a coordinated approach.
