Advancing Health Equity in State and Territorial Health Departments – This infographic explores why state and territorial health departments should advance health equity and offers activities in each chronic disease domain that health departments can take to address health equity. (GEAR Group 5, 2017)

Communicating the Value of Public Health – This product is intended to aid you in communicating with legislators about the value of public health. It is recognized, that as a public employee, there are policies and restrictions around such activity. Some of the recommendations may fit within the scope of what is allowable, and some may not. Before engaging in legislative communication or activity, you are encouraged to check in with your Government Affairs Officer. You are also encouraged to focus on state and local issues; the National Association of Chronic Disease Directors will continue to foster and maintain relationships with federal authorities and members of Congress centrally in order to ensure consistent messaging at the national level. Communicating the Value of Public Health through Data and Surveillance – Members of this group shared experiences in using quantitative data, qualitative data, and epidemiological methods to help decision makers, project partners, and other groups understand the value of chronic disease data and evidence-based public health approaches.  The GEAR Group 1 product presents examples from seven state health departments and an infographic describing how epidemiology and surveillance contribute to the value of public health across the four chronic disease domains and health equity. (GEAR Group 1, 2017)

Payment Reform Matrix – The United States healthcare system is changing rapidly, placing greater emphasis on primary care and transitioning from an acute care model to comprehensive chronic disease management, which can and should include integration with public health. The National Payment Reform Initiatives – Related to Chronic Disease Prevention Matrix is intended as a resource for public health practitioners interested in understanding specific national payment reform initiatives and how some state chronic disease prevention programs have been involved in these initiatives. One of the 2016 GEAR Groups focused on Understanding Healthcare Payment Reform and created this matrix to facilitate cross-learning between health departments and the sharing of new ideas. It can also be used as a tool for orienting public health professionals new to working with healthcare systems. (GEAR Group 3, 2016)

Community Benefit InfographicThis product provides information and best practice examples on the ACA’s requirement for community benefit for non-profit hospitals and other organizations and its linkage to the Triple Aim.  Requirements include a community health needs assessment and community health promotion activities.  State and local public health organizations can use this information to understand ways to support and partner with these organizations in meeting the community health needs assessment requirement and collaborating on health promotion activities. (GEAR Group 2, 2016)

Understanding Chronic Disease Data and Methods for Communicating Impact – This group explored chronic disease data, levels of evidence and methods for analyzing on data visualization, return on investment, working with media, small area estimation, health system quality improvement initiatives, and other topics selected by the GEAR Group. The group prepared a document for NACDD’s Chronic Disease Practitioner Network providing examples of state uses of chronic disease data, including demonstrating return on investment. The GEAR Group members reviewed published literature and explored professional development recommendations for chronic disease and health promotion staff. (GEAR Group 1, 2016)

The Value of Clinical-Community Linkages in Healthcare – The GEAR Group developed a template that serves as a general guideline to open dialogue with clinicians to provide insight into the benefits of their participation in clinical-community linkages. The information was purposely left broad as to allow agencies utilizing this tool to substitute more specific examples of programs, benefits, etc. as they relate to the clinical-community linkages available in their state or territory. The Value of Policy, System, and Environmental Change in Public Health – This fact sheet walks through the Health Impact Pyramid, emphasizing the importance of PSE change and encourages public health professionals to invest resources into these. (GEAR Group 2, 2017)

The Value Public Health Offers Health Systems – Members of this GEAR Group chose to focus on the value that public health brings to health systems as it relates to the chronic disease domains defined by CDC’s National Center for Chronic Disease Prevention and Health Promotion as well as health equity. Available as either slides or an infographic. (GEAR Group 3, 2017)
