Celebrating Change highlights the important work of communities to make the healthy choice the easy choice for the millions of people at risk for chronic diseases such as heart disease and diabetes. Chronic diseases are among the most common, costly and preventable health problems in the U.S. To celebrate the hard work of the funded communities and the ACHIEVE national partnership, NACDD shares this collection of success stories from across the nation, providing a snapshot of how this viral work made an impact in local communities. NACDD and the ACHIEVE national partnership continue to promote the ACHIEVE philosophy of creating healthier communities through sustainable change within community settings - a philosophy that saves lives, improves quality of life, and reduces healthcare costs.
Winter Park Health Foundation Case Study
This document details the Winter Park Health Foundation's (WPHF) efforts to implement sustainable health strategies using community leadership approaches derived from the Action Communities for Health, Innovation, and EnVironmental changE (ACHIEVE) -- Healthy Communities model. It highlights successes, challenges, and provides ample consideration for how health foundations or community foundations, can accelerate the Healthy Communities model within local settings so healthy choices become the easy choices where people live, learn, work, worship, receive care, and play.

The "NACDD Communities ACHIEVEing Success" document is a compendium of information that showcases healthy community changes of NACDD funded ACHIEVE communities in the areas of increasing physical activity and healthy eating, reducing or preventing tobacco use, chronic disease management, and leadership.
The "Creating Healthy Communities - Success Stories from ACHIEVE" is a document about the impact of healthy community policy and environmental change efforts around the country between 2008-2012.
Peer Learning Network Report
In 2009-2012 NACDD developed a Peer Learning Network to support the community teams working to create healthy communities. "This report" outlines the process and outcome of that effort.
