Healthy Communities Program
CDC's Healthy Communities Program works with communities
through local, state and territory, and national partnerships to improve community
leaders and stakeholders' skills and commitments for establishing, advancing,
and maintaining effective population-based strategies that reduce the burden of
chronic disease and achieve health equity. Communities create momentum that
assists people in making healthy choices where they live, learn, work, and play
through sustainable changes that address the major risk factors—tobacco,
physical inactivity, and unhealthy eating. Currently, 331 communities and 52
state and territorial health departments have been funded.
Communication Tools: This section includes "tools and resources" for supporting healthy community social
media outreach, sample communication pieces (Press releases, media advisory,

Community Mentor Model: This informative document emphasizes the principles and successes of the Community Mentorship Model (CMM) as an innovative framework of community partnership between and among states for collaborative efforts to improve community health.
Healthy Communities Multisectoral Resource Compendium
CDC and its plethora of national partner organizations have collectively and individually produced numerous tools, products, and publications detailing these Healthy Communities efforts, and documenting successes, lessons learned, and the multisectoral community processes from around the nation. This document serves as a compendium of such resources, and is intended for chronic disease prevention practitioners across all public health levels that want to broaden their evidence and practice-based knowledge of the use of the multisectoral model in creating healthier communities. More than 80 resources are categorized in the below charts according to topic area of interest, complete with the authoring national partner organization(s), resource summaries, and access links.
