CDC's Healthy Communities Program works with communities through local, state and territory, and national partnerships to improve community leaders and stakeholders' skills and commitments for establishing, advancing, and maintaining effective population-based strategies that reduce the burden of chronic disease and achieve health equity. Communities create momentum that assists people in making healthy choices where they live, learn, work, and play through sustainable changes that address the major risk factors—tobacco, physical inactivity, and unhealthy eating. Currently, 331 communities and 52 state and territorial health departments have been funded.
Communication Tools: This section includes "tools and resources" for supporting healthy community social media outreach, sample communication pieces (Press releases, media advisory, etc.)
Podcasts: These "Podcasts" short recorded audio productions – highlight NACDD Healthy Communities across the nation. Simply click on the links to hear the podcast or right-click your mouse and "Save link as” on your computer or MP3 player. Success Stories: NACDD has compiled a number of "Success Stories" from all over the country demonstrating the potential communities have to be successful in creating healthy communities.
Sustainability Resources: Comprehensive resource guide "Sustaining the Movement" to support continued momentum among healthy communities. Sustainability webcasts were designed to encourage healthy communities to have a continual dialog about how to change lives in your community.
Community Mentor Model: This informative document emphasizes the principles and successes of the Community Mentorship Model (CMM) as an innovative framework of community partnership between and among states for collaborative efforts to improve community health.
