    Data-Driven Systems Change and Improvement in Hypertension Management

This issue brief summarizes the NACDD fireside chat focused on health systems changes implemented through a partnership between the Massachusetts Department of Public Health and its Health Center Controlled Network, the Massachusetts League of Community Health Centers. Moderated by Sallyann Coleman King, MD, MSC, CDC medical epidemiologist, the fireside chat featured Anita Christie, RN, MHA, CPHQ, director of the Office of Clinical Preventative Services with the Massachusetts Department of Public Health discussing the MDPH approach to 1422 planning through a detailed capacity assessment process and ongoing quality improvement work with selected health centers. Diana Erani, MBA, Health Center Controlled Network director for the Massachusetts League of Community Health Centers provided an overview of the Data Reporting and Visualization System (DRVS) used to deliver feedback to health centers in collaboration with MDPH and shared her perspective on institutionalizing data-driven technical assistance.

    Addressing Health Disparities in Hypertension Control

This issue brief summarizes the NACDD fireside chat and virtual roundtables with staff from the South Carolina Department of Health and Environmental Control (DHEC) and featured their longtime partner, Dr. Brent Egan, senior medical director of the Care Coordination Institute(CCI). DHEC has made health disparities a priority in its work to improve the lives of South Carolinians and works with many partners as part of its 1305 and 1422 CDC cooperative agreements. Several themes emerged from the discussions including the role of data in identifying areas or populations with high rates of hypertension; the need for a deliberate intention to address health disparities and achieve health equity when providing technical assistance; and the importance of working with partners who share your mission.

    Team-Based Approaches to Controlling Hypertension

NACDD and the CDC’s Division for Heart Disease and Stroke Prevention hosted a fireside chat and follow-up virtual roundtables to address team-based approaches to controlling hypertension. Moderated by the Million Hearts Executive Director, Janet Wright, this chat focused on the pharmacist-related work that Connecticut has been involved with. During the peer-to-peer virtual roundtables, several states shared their experience in promoting team-based care, including Alaska, Iowa, Nebraska, Nevada, Utah, Vermont, Wisconsin, Washington, and Colorado.  

 

Undiagnosed Hypertension
To support the Million Hearts focus of identifying people with undiagnosed – and therefore untreated – hypertension,  NACDD and the CDC’s Division for Heart Disease and Stroke Prevention launched a series of learning opportunities in February 2016, including a fireside chat held at CDC and live streamed, two virtual roundtables to allow grantees to discuss in more detail their work related to undiagnosed hypertension, and the development of success stories featuring strategies that state health departments have implemented. A virtual roundtable summary document provides highlights from the fireside chat and the roundtables and case studies from Colorado, Maine, Massachusetts, Montana, Texas and Wisconsin. The issue brief highlights lessons learned and success stories shared by New York, Florida, and Montana about their work related to undiagnosed hypertension.
