The CVH Network Leadership Team is accepting
nominations!

Terms will begin in October 2016 and continue through
September 2017. Please complete
this form to nominate yourself or your Domain 3 colleagues.

Nominations are due Friday, September 30, 2016.

Please contact MaryCatherine Jones,
mcjones@chronicdisease.org, with questions
regarding the Leadership Team and nominations process.

What is the CVH Network Leadership Team?

The CVH Network Leadership Team is a group of state health department staff working in Domain 3, as well as committee and partner liaisons, who

  • Represent state health departments to NACDD, CDC, and other partners,

  • Set the priorities and best methods for providing support to state health departments, such as recommending topics for webinars and Fireside Chats and helping to determine the most suitable means to disseminate information, and

  • Coordinate and integrate activities with other NACDD councils and committees.

The CVH Network Leadership Team provides extensive input and feedback to NACDD on our learning opportunities, the Off the Cuff newsletter, and other health systems/Domain 3 activities.

Who can be nominated?

Any state health department staff working in an area related to Domain 3 is eligible to be nominated, provided that they are generally able to attend the scheduled monthly CVHN Leadership Team meetings for the full term of the position (October 2016-September 2017). Nominees do not need to work exclusively in Domain 3. New staff and nominees from states not currently represented on the Leadership Team are encouraged to apply (see the list of returning 2016-2017 Leadership Team members below). Self-nominations are welcome.

What is the time commitment?

Approximately 1 hour per month. CVH Leadership Team meetings are held monthly on the fourth Wednesday at 1pm ET. Between calls, members may be asked to review agendas and/or minutes. Occasionally, members are asked to provide input on documents between calls. Some positions require additional time. Position descriptions are available below.

What are the benefits of serving on the CVH Leadership Team?

  • Peer networking, communication, and problem solving on common issues

  • Opportunities to inform the development of training and resources for states related to Domain 3

  • Opportunity to develop or refine a national perspective, contribute to CDC and NACDD understanding of the state context for Domain 3, and be a voice for other states

What happens after I submit a nomination?

NACDD will confirm the nomination with the nominees, collect any additional biographical information, and announce the slate of candidates via Off the Cuff with a link to the electronic ballot. All CVH Network members are encouraged to participate in the election.
