National Diabetes Prevention Program :: Michigan


The Michigan Diabetes and Prevention Program (DPCP) has established a statewide network of partners to address diabetes prevention and building system change to assure referrals of high risk adults to evidence-based lifestyle programs, such as the National Diabetes Prevention Program (NDPP). The Michigan Diabetes Prevention Network is comprised of partners delivering the NDPP, as well as national and state partners working to promote the NDPP.  The DPCP convenes the network partners, and shares information and resources.  To address sustainability of the NDPP, the DPCP is working with seven state partners to impact system change by establishing formal referral mechanisms at the provider or health system level to assure referrals to local diabetes prevention programs.
