National Diabetes Prevention Program :: Minnesota

Minnesota

In an effort to prevent or delay the onset of type 2 diabetes, Minnesota is raising awareness of prediabetes, its risk factors, and how providers can recognize and treat prediabetes using evidence-based National Diabetes Prevention Program lifestyle programs. In order to raise awareness with providers, Minnesota designed a webinar for health providers regarding prediabetes awareness and diabetes prevention. In conjunction with provider awareness, the MN Diabetes Prevention and Control Program (DPCP) updated its clinical algorithm and provider tool kit to increase provider referrals to evidence-based lifestyle programs.

The DPCP also created culturally specific awareness campaigns in partnership with several communities including: African American, American Indian, Latino, Hmong, and Somali communities.
