Introduction to the CDC Health Systems Scorecard Quality Improvement Assessment Tool

Using a scorecard approach, CDC developed a voluntary quality improvement assessment tool called the Health Systems Scorecard to help CDC-funded state and local public health programs better understand the implementation of evidence-based strategies to manage chronic diseases among partnering primary care practices. Presenters will provide an orientation and demonstration of the electronic tool and its primary components, describe how to set up an account, discuss approaches to inviting health systems to participate, and the benefits of implementing the Scorecard. Leveraging an EHR Querying System and Other Existing Data Sources to Monitor and Report on Patient Quality of Care

These calls generally consist of a presentation from an awardee on a specific topic, followed by small group breakout discussions to facilitate sharing of experiences, successes, challenges and lessons learned. Audio, Slides, Assessment Report, and DRVS (Data Reporting and Visualization System) for 1422 Measures from the June, 2017 Fireside Chat on Addressing Health Systems Change in Hypertension Control Are Available

This fireside chat focused on addressing health systems change in hypertension control. Ms. Christie, RN MHA CPHQ, the director of the Office of Clinical Preventative Services shared her experience designing Massachusetts’ 1422 approach through a detailed capacity assessment process as well as their ongoing quality improvement work with sites. Ms. Erani, MBA, Health Center Controlled Network Director for MLCHC, provided an overview of the Data Reporting and Visualization System (DRVS) used to deliver feedback to 1422 clinical sites.
