The Chronic Disease GIS Network, sponsored by the CDC Division for Heart Disease and Stroke Prevention (DHDSP) in collaboration with NACDD, facilitates networking among health department staff and with CDC regarding the use of GIS to address chronic disease surveillance, prevention and management. This webinar will feature presentations of maps created by health department staff working with GIS for chronic disease prevention. Fireside Chat: Addressing Health Systems Change in Hypertension Control

This Fireside Chat will focus on addressing health systems change in hypertension control. The session will be moderated by CDC Medical Epidemiologist Sallyann Coleman King, MD, MSC. It will feature Anita Christie,
RN MHA CPHQ, Director of the Office of Clinical Preventative Services from the Massachusetts Department of Public Health, and Diana Erani, MBA, Health Center Controlled Network Director for the Massachusetts League of Community Health Centers (MLCHC). Ms. Christie will share her experience designing Massachusetts’ 1422 approach through a detailed capacity assessment process as well as their ongoing quality improvement work with sites. Ms. Erani will provide an overview of the Data Reporting and Visualization System (DRVS) used to deliver feedback to 1422 clinical sites in collaboration with the Massachusetts Department of Public Health and will share her perspective on institutionalizing data-driven technical assistance. From NACDD Diabetes and CVH Teams
Become a Peer-to-Peer Guide!

The CVH Network and Diabetes Council are recruiting guides to participate in the next cohort of the Peg Adams Peer-to-Peer Program, which will begin September 2017. The mission of the Program is to provide support, encouragement, and orientation to new Diabetes and CVH state staff. The program will be from September 2017 to June 2018. The level of engagement may vary, however, we anticipate this to be at least a 2-3 hour/month commitment.

We highly encourage all state staff, including epidemiologists and evaluators, with 3 or more years experience to consider serving as a guide.
