The CDC Health Systems Scorecard: A Quality Improvement Tool to Assess Health System-Level Policies for Managing Patients with Chronic Disease

Using a scorecard approach, CDC developed a voluntary quality improvement assessment tool called the Health Systems Scorecard to help CDC-funded state and local public health programs to better understand the implementation of evidence-based strategies to manage chronic diseases among partnering primary care practices. Ultimately the tool may assist to 1) help establish relationships between public health departments and primary care practices, and 2) provide standards for assessment of evidence-based care management policies and practices. Presenters will provide a brief orientation to the electronic tool and its primary components. Scorecard modules include health system policies that support use of multi-disciplinary care teams; clinical guidelines; electronic functions for managing patient health and follow-up; clinical decision supports; patient education and self-management; tobacco use cessation; and screening for breast, cervical, and/or colorectal cancer. Leveraging Utilization of Electronic Health Records

This month’s call will provide an opportunity to discuss leveraging utilization of electronic health records (EHRs). Presenters are: Karen Fuller, Public Health Consultant, Michigan Department of Health and Human Services, and Bruce Maki, Project Manager, Michigan Center for Effective IT Adoption (M-CEITA). How to participate on the call:

You will be able to hear the audio when you join Adobe Connect, but in order to actively participate you must deliberately join your phone line. After you join the Adobe Connect platform, you will be prompted to select an audio option. You can either:

  • Have the system call you (preferred method): When you first enter the meeting room during the scheduled time, enter your phone number, and let the Adobe system call you. You will then be prompted to press a number to join the meeting.
  • Dial in to the audio conference manually: When you first enter the meeting room during the scheduled meeting time, you will see a menu box with an option to dial in to the audio conference via phone. Select that option and dial in using the number and code provided. You MUST enter an additional code after connecting with the passcode so that the system ties your computer to your audio line. You must scroll down to see your unique code. Your unique code will appear after the following statement: “Once joined to the audio, identify yourself.”

Developing Strong Health Impact Statements

As grantees report on their final results of 1305 and 1422 programs, it will be important for them to demonstrate the difference they are making in their states and communities. As part of their evaluation efforts, grantees will be required to report on their progress toward achieving health outcomes and the value of their efforts to the well-being of the populations they serve. A health impact statement can be an important tool for meeting these evaluation and communication needs. This webinar will explain what a health impact statement is, the key elements of a strong health impact statement, and how to use a health impact statement. This webinar will also outline CDC’s expectations for the final year of evaluation reporting.

By the end of this Webinar, participants will:

  • Know the definition of health impact and health impact statement,
  • Be able to identify the key elements of a strong health impact statement,
  • Understand how health impact statements are used,
  • Be able to identify strategies for which they can plan to develop health impact statements, and
  • Understand the evaluation reporting expectations.
