Making the Most of Your Program Logic Model

Join us for a 20-minute coffee break on innovative ways to use a logic model for program planning and evaluation. Logic models are an often misunderstood, yet important tool for program planning and evaluation. More than just a series of columns and arrows, the process of creating a logic model can help public health programs elucidate their underlying theory of change and refine their ‘elevator pitch’ to describe to others the program and its value. Logic models are an important tool to focus local program evaluations, identify evaluation questions and indicators, and report findings. This presentation will illustrate how a simple ‘road map’ logic model can serve multiple purposes. Using a DHDSP program logic model as an example, participants will gain insight into how a logic model can be used for planning and evaluation throughout a program’s lifecycle. Call for Applications: Advancing Team-Based Care Through the Use of Collaborative Practice Agreements and Using the Pharmacists’ Patient Care Process to Manage High Blood Pressure


The National Association of Chronic Disease Directors (NACDD), in coordination with CDC’s Division for Heart Disease and Stroke Prevention, is pleased to announce an opportunity for teams to participate in a program designed to accelerate team-based care to manage high blood pressure, through the use of the Pharmacists’ Patient Care Process (PPCP) and Collaborative Practice Agreements (CPA) resource guides developed by CDC and pharmacy partners.

NACDD and CDC will facilitate a learning collaborative for 4-6 selected teams comprised of state and/or local 1305, 1422, or WISEWOMAN grantees along with a pharmacist association representative. These teams will serve as facilitators-in-training to acquire the information needed to effectively host workshops within their state or city for independent community pharmacists and primary care physician members.
