Successful Implementation of Million Hearts Data Strategies to Reduce Blood Pressure

The District of Columbia, Department of Health Chronic Disease Division leads a District-wide Million Hearts Learning Collaborative. They established data sharing agreements to facilitate the collection of electronic health record aggregate data for 10 health systems and compile these data to create broad population-level metrics used for monitoring purposes and tracking program progress. DC Million Hearts population-level monitoring data now includes all the Federally Qualified Health Centers (FQHCs) in DC.

Webinar Description: Integrating Community Health Workers (CHWs) into the public health and healthcare workforce is a growing strategy for increasing health equity and access to improve population health, while decreasing costs. ASTHO, with funding from HRSA (Health Resources and Services Administrations of the US Department of Health and Human Services), hosted an interactive panel discussion to highlight issues related to evaluating CHW programs. This webinar provides an overview of CHW evaluation, describes the CHW Common Indicators Project, and highlights action steps that participants can take to capture the contributions of CHWs in their individual settings.
