New Health Systems Quality Improvement Tool

The new CDC Health Systems Scorecard tool helps health systems assess evidence-based chronic disease care management policies and practices. State and local health departments can share the scorecard tool with their health systems colleagues to stimulate clinical-public health partnerships. In a July NACDD webinar, presenters provided an orientation to and demonstration of the electronic tool’s primary components, described how to set up an account, discussed approaches to inviting health systems to participate, and reviewed the benefits of implementing the scorecard.
