NACDD is the only organization representing all state and jurisdictional chronic disease staff, dedicated to building capacity and making public health good for the public.
Message from the CEO
The end of August typically announces the end of summer and a reprieve from the heat.  As kids go back to school, we know that the temperature in most state health departments is heating up for the end of the fiscal year.  This can cause a lot of stress and hand-wringing.  In the event that your state or territory needs help obligating monies for this year or planning for next fiscal year, I invite you to contact me directly so that we can discuss the many options that NACDD has to provide your state with training, Technical Assistance, professional development, evaluation or communication services.  As you know, NACDD provides a number of in-person, state assessment tolls such as State Technical Assistance and Review Program (STAR) and Policy State Technical Assistance Team (PSTAT).  If you would like to ask questions about any of these or if you would like to discuss the possibility of hosting a State-Based Chronic Disease Academy, we can help you think through the various options available to you.  NACDD recently conducted the Wyoming Chronic Disease Academy with curriculum tailored around 1305 objectives and requirements.  As a reminder, NACDD is here to serve you.  Please let us know how we can bring your fiscal year to a successful close.

NPHIC Designs Immunization Awareness Toolkit
August is National Immunization Awareness Month and the National Public Health Information Coalition has designed a toolkit to communicate the importance of immunizations during this period of observance and throughout the remainder of the year.  The immunization awareness toolkit contains key messages, vaccine information, sample news releases and articles, sample social media messages and links to web resources from CDC and other organizations.  Click here for more information.
Save the Date:  AHRQ/PCORI Nursing Webinar
On September 10, 2014 from 2:00 - 3:00 p.m. the Agency for Healthcare Research and Quality will co-host a Webinar with the Patient Centered Outcomes Research Institute (PCORI).  This Webinar will include an overview of the EHC Program and PCORI, highlights of each organization's research projects, and ways in which nurses can get involved with research conducted by both organizations.  In addition, participants will learn about key findings from one of the EHC Program's CER reviews, Non-Surgical Treatments for Urinary Incontinence in Adult Women:  Diagnosis and Comparative Effectiveness.  Registration is free bust space is limited.  To register and receive information to access this Webinar, please click here.
Register for the National Forum's 12th Annual Meeting
The National Forum for Heart Disease and Stroke Prevention's 12th Annual Meeting will be held October 22, 2014 in Washington, DC.  The meeting will include National Forum members sharing success stories on impactful collaborative initiatives, population health data and best practices, reducing disparities in heart disease and stroke, best practices in blood pressure control and opportunities and challenges to cholesterol control.  Read more
New Resource from the National Diabetes Education Program (NDEP) Can Help States With 1305 Health System Interventions
This Practice Transformation site, formerly called Better Diabetes Care, is designed to help physicians, health care professionals and health care administrators across the country who want to change the system of health care delivery around diabetes.  NDEP believes that practice change is essential to providing the type of evidence-based care recommended by the Patient-Centered Medical Home (PCMH) model to effectively manage diabetes, prevent its serious complications, and delay or prevent type 2 diabetes. The PCMH incorporates dimensions of patient-centered care presented by the Institute of Medicine and the Chronic Care Model.  This site is organized according to those dimensions.  Read more
