Communities of Practice Forming

The Coordinated Chronic Disease Learning Community is sponsoring two Communities of Practice that will explore the issues, needs, and potential resources related to cross training chronic disease staff and implementing chronic disease self-management interventions. These Communities of Practice will begin meeting shortly and will continue for 6 months. There are limited seats available. Organizational Self Assessment for Addressing Health Inequities - Pilot Project

Ahead of the curve! Four state health departments (OR, NC, IA, and VA) are participating in a pilot project to revise the Organizational Self Assessment for Addressing Health Inequities Toolkit and Guidance. Each state has agreed to apply the five tools in the kit and provide feedback based on their experiences. Their recommendations will be incorporated into the final version, due to be released in October 2013.

CDC's Chronic Kidney Disease Initiative Announces the Launch of Its New Surveillance System Web Site

The Centers for Disease Control and Prevention’s Chronic Kidney Disease (CKD) Initiative announces the launch of the new CKD Surveillance System Web site,, which provides fast facts and surveillance trends. Be sure to share this useful resource with your colleagues and partners.

Update on HOPE

NACDD was recently funded by the CDC to implement the HOPE (Health Opportunities through Prevention and Education) Program. In this project, NACDD will work with the CDC's Division of Community Health to translate NACDD's (and other national partners!) Healthy Community experience so that other communities, states, and organizations can benefit from learning how funded communities have been successful at implementing policy, system, and environmental changes to create a healthier commuity and prevent chronic diseases.

NACDD's Work with the Diabetes Leadership Initiative in Madison, Wisconsin

Wisconsin's Madison Magazine, which reaches over 20,000 readers, hailed NACDD's work with the Diabetes Leadership Initiative in their latest issue. The article reported, "the collaboration between state health officials, clinicians and diabetes experts at the local and national levels has allowed the Diabetes Leadership Initiative to grow, and NACDD has been "extremely supportive" of the work here in Wisconsin".

Each year, NACDD recognizes individual members of its Association for outstanding work in the field of chronic disease control and prevention. Any state health department employee serving in the area of chronic disease is eligible. Click the link below for descriptions or the awards.

NACDD to Move National Headquarters

NACDD will be moving it's Atlanta-based headquarters down the street to a new facility in mid-January. The move will only affect NACDD's mailing address. All other contact information such as email, phone and fax will remain the same.

The Impact of Social and Economic Factors on Health

Dr. Brian Smedley of the Health Policy Institute at the Joint Center for Political and Economic Studies will provide a general orientation to demonstrate how social and economic factors affect health outcomes using initiatives like PLACE MATTERS. He will also describe selected case studies of situations where social and economic factors were applied to measure progress using examples like Moving to Opportunity, and recommend opportunities for NACDD members to partner with organizations like the Joint Center for Political and Economic Studies. As a result of the Webinar, participants will recognize the impact of an intervention that addresses a social or economic factor resulting in a positive health outcome.
