Approved Vendor Application

As a federally funded, nonprofit organization, the National Association of Chronic Disease Directors (NACDD) has specific requirements and processes for using external vendors to assist with or complete our program activities. The Communications and Member Services Department identifies and approves vendors to work in close collaboration with staff and consultants. View the list of Approved Vendors here.

To request status as an NACDD-approved vendor, businesses are asked to complete the below form and email it to Please allow 30 business days for a response.
