Emerging Practices in Diabetes Series State health departments are exploring and testing new and innovative approaches that will provide critical insight and lessons learned. We refer to these approaches as emerging practices. The goal of the Emerging Practices in Diabetes series is to summarize and share information on these proven practices to inform the work of other states, cities, or communities working in the same or related areas under 1305 and 1422.

Find a list of strategies and performance measures related to community programs linked to clinical services for each of the cooperative agreements on the left side bar.
