SELF-MANAGEMENT CHRONIC DISEASE SELF-MANAGEMENT PROGRAM

EVIDENCE

Arthritis Self-Management Program (ASMP)/Chronic Disease Self-Management Program (CDSMP) Meta-analyses were conducted to examine the specific effects of two self-management education programs developed at Stanford University, CDSMP and ASMP. Each meta-analysis combined the results of more than 20 studies of the intervention. An Executive Summary of the full report "Sorting through the Evidence for the Arthritis Self-Management Program and the Chronic Disease Self-Management Program" is available.

The CDC produced two issue briefs based on the results of the above mentioned meta-analyses. One issue brief is designed for organizations that are considering adopting CDSMP and/or ASMP, with a key message that adopting these interventions can help members of the community thrive. A second brief is designed for health care providers with the key message that recommending these evidence-based community programs can complement clinical care and help chronic disease patients live happier, healthier lives.
