SELF-MANAGEMENT CHRONIC DISEASE SELF-MANAGEMENT PROGRAM
The Stanford University Patient Education Research Center
For over 3 decades Stanford University has developed, evaluated, and translated into practice self-management programs for English and Spanish-speakers with chronic health problems, as well as cancer survivors and caregivers. A Stanford University study completed in 1996 provided the basis for the development of the Chronic Disease Self-Management Program. The Stanford Patient Education Research Center also produces the Diabetes-Self Management Program for people with diabetes.
