NACDD Awards $40,000 to Communities in Florida and Michigan to Advance EBSM Programs

NACDD is pleased to announce that four community providers in Florida and Michigan have been selected to receive funding to advance evidence-based self-management programs.  These competitive mini-grants will provide $10,000 to each community provider to enable local action to address multiple chronic conditions, specifically arthritis and the chronic disease risk factor of physical inactivity at the policy, systems, and environmental changes.


The following organizations will collaborate locally and support instructor training, participate in recruitment and marketing to provide training programs to people living with arthritis in their respective communities.


·      The Area Agency on Aging of Western Michigan located in Grand Rapids

·      Miami-Dade Parks, Recreation and Open Spaces Department located in Florida

·      Detroit, Michigan Area Agency on Aging

·      Great Lakes Center for Health Innovation, (a division of the National Kidney Foundation of Michigan)


Arthritis impacts the daily activities of over 21 million Americans.  People with arthritis are less likely to be physically active which increases the likelihood of developing other chronic diseases and conditions—specifically obesity.  Providing enhanced opportunities to be active to people with arthritis has been demonstrated to improve their overall health and quality of living.
