2019 Impact Awards Nominations

Nominations close Monday, Jan. 21 at 11:59 p.m. ET.
For more than a decade, state, tribal, and territorial Chronic Disease Directors have used the Impact Awards to acknowledge the exceptional work and progress chronic disease professionals and partners are making in the field of public health. The National Association of Chronic Disease Directors oversees the administrative responsibilities of the awards process, but the awards are decided by NACDD Members.

NACDD’s annual Impact Awards recognize exceptional public health practitioners, State, Tribal, and Territorial Health Departments, and partners. The NACDD Awards Committee, which includes NACDD Members and Headquarters staff, receives and judges all nominations.

This year, as part of celebrations for NACDD’s 30th anniversary, several special award categories have been added. Recipients will be honored during an awards reception at the 2019 Chronic Disease Academy in April
in Atlanta (either in person if they are already part of the state team traveling to the Academy or in absentia). Award nomination qualifications are listed below. You also may nominate yourself.
The NACDD Impact Awards are open to all NACDD Members listed in our Member roster. To be an NACDD Member, you must be a public health professional working in chronic disease prevention and control. More information on NACDD Membership is available at http://www.chronicdisease.org/page/ForMembers.
Awarded to an individual outside the traditional public health field who has made outstanding contributions in the field of chronic disease prevention and control.
Awarded to an individual or team that has made a significant impact or achieved significant influence in a community related to chronic disease prevention and control.
Awarded to a state, tribal, or territorial Chronic Disease Unit that demonstrates an innovative approach to reducing the burden of chronic disease.
Awarded to an individual staff member in a state, tribal, or territorial chronic disease unit who is not a director and who has demonstrated exceptional leadership and innovation to promote health.
Awarded to an outstanding chronic disease prevention and control professional who has guided, supported, and promoted the training and career development of other chronic disease prevention and control practitioners working in a State Health Department. Nominees should have a sustained record of mentoring over time and nominators must be mentees of the nominee or colleagues who have personal knowledge of the nominee’s mentoring efforts.
Awarded to a current or former Chronic Disease Director of a State, Tribal, or Territorial Health Department whose career has contributed to reducing the burden of chronic disease in the United States.
1) Only one person may be recognized per accomplishment, only one award nomination category per program or person will be considered.
2) Form letters are discouraged.
3) State/Tribal/Territorial Chronic Disease Departments are encouraged to submit nominations as a unit and with the approval of the Chronic Disease Director.






Please provide your application in one PDF document, no larger than 30 MB in size, which will include the following:
1. A letter of support detailing why the nominee is being nominated, the problem or challenge the nominee faced and the accomplishments of the nominee.
2. The resume(s) of the nominee, including previous awards received and any relevant publications history.
3. Links to videos, images, or other supplementary materials that can help further elaborate on the nominee's accomplishments.
