Topics chosen are based on needs assessment data from NACDD membership and aligned with the NACDD Chronic Disease Competency domains. Speakers represent a variety of partner and national organizations. These webinars are presented monthly, on a one-hour webinar platform on the fourth Thursday at 3:00 p.m. EST.

Dissemination and implementation (D&I) science and their basis in diffusion of innovations research and practice have led to a number of evidence-based lessons about how to effectively spread evidence-based practices and programs. The converse is also true -- we have a good idea from theoretical and empirical work about what NOT to do in trying to encourage adoption and support effective implementation of evidence-based practices and programs. This presentation will focus on upstream targeting; that is, a focus on direct service providers (nurses, physicians, clinicians etc) who work with populations at risk or managing chronic diseases.

Jim Dearing is Professor and Chairperson of the Department of Communication at Michigan State University. Dearing was Senior Scientist for Kaiser Permanente, where he was Principal Investigator of a National Cancer Institute center of excellence. Dr. Dearing studies diffusion of innovations, and use of diffusion principles to disseminate, implement and sustain worthy innovations. He works and speaks with teams to accelerate the spread of evidence-based practices, programs, policies, and technologies, and measure diffusion. He studied under and worked with Everett M. Rogers for 20 years. Dearing has led projects funded by the Bill & Melinda Gates Foundation, the John D. and Catherine T. McArthur Foundation, the Robert Wood Johnson Foundation, the W.K. Kellogg Foundation, and U.S. federal agencies. His most recent book is Health Communication Research Measures (Peter Lang, 2016). His current study of the scale up of 45 social innovations is funded by The Wallace Foundation.

NACDD, in coordination with CDC’s Division for Heart Disease and Stroke Prevention and Million Hearts®, is proud to launch the first Fireside Chat of 2016. Fireside Chats will provide a new, interactive format for state health agencies to hear from content experts on a number of issues pertaining to cardiovascular health and 1305 priority areas. The first Fireside Chat will highlight innovative strategies on identifying patients with undiagnosed hypertension. Speakers will include Dr. Ian Brissette from the New York State Department of Health, and Dr. James Schultz from Neighborhood Healthcare in San Diego, CA.  We will be live streaming as the speakers present on their experiences, engage in informative dialogue, and we will also open the lines to allow for questions and perspective from the audience.  Contact Julia Schneider, jschneider@chronicdisease.org, with questions.

Objectives for the Fireside Chat

  • Provide an interactive, unique format for adult learning.
  • Highlight the state health department role in providing technical assistance to healthcare partners on identifying patients with undiagnosed hypertension.
  • Identify various data sources and algorithms to identify undiagnosed hypertension.
  • Learn about the successes and challenges from a community health center who collaborated with their local health agency to identify and follow up with undiagnosed patients.
