Cardiovascular Health

           

The focus of the NACDD cardiovascular initiative is the Cardiovascular Health Network, comprised of health department program staff, epidemiologists, and evaluators working to improve diagnosis and management of hypertension through health systems.

The Network supports health departments in implementing population health improvement strategies in healthcare systems which are primarily associated with CDC’s Division for Heart Disease and Stroke Prevention-funded activities in the 1815 and 1817 grants.
