Creative Product and Media Outreach Request Form

The Communications Department oversees the promotion of NACDD’s work by providing full-service development and review of publications, newsletters, fliers, brochures, videos, other creative products, and offers assistance with outreach to journalists.

Use this form to:

  1. Ask the Communications department to review and approve a document/video that was created without Communications department support.  
  2. Ask the Communications department for assistance developing a new document/video. (Materials the Communications department produces do not need to use this form for final review.)
  3. Request support and approval for pro-active public relations activities, such as press release drafting, responding to a request from a journalist, and developing a publicity strategy for activities or program milestones.

For urgent matters, such as when a journalist directly contacts a staff member or consultant, contact the Director of Communications, Paige Rohe, prohe@chronicdisease.org, to receive assistance with managing and responding to the inquiry. 

Where to get more help:  

After form submission, the Communications Department will respond to the request and provide an action plan for moving forward within three business days, as possible.
