Medication Therapy Management (MTM) is care provided by pharmacists whose aim is to optimize drug therapy and improve therapeutic outcomes for patients. Medication therapy management includes a broad range of professional activities, including but not limited to (1) performing patient assessment/comprehensive medication review, (2) formulating a medication treatment plan, (3) monitoring efficacy and safety of medication therapy, (3) enhancing medication adherence through empowerment/education, and (4) documenting and communicating MTM services to prescribers. This presentation will further define MTM services, how MTM services improve outcomes, barriers to providing MTM services, and steps to increase pharmacist-provided MTM services to high-burdened populations.
