CDC and NACDD Fireside Chat: Cholesterol and Cholesterol Management

CDC Division for Heart Disease and Stroke Prevention and the NACDD CVH Team are hosting the next Fireside Chat focused on cholesterol and cholesterol management. Fireside Chats provide an interactive format for state health agencies to hear from content experts on a number of topics pertaining to cardiovascular health and the DP18-1815/DP18-1817 priority areas. This Fireside Chat will focus on:
  • Defining cholesterol
  • Epidemiology of cholesterol in the United States
  • 2018 Guidelines for Management of Blood Cholesterol 
  • Cholesterol clinical quality measures
  • Team-based care strategies for cholesterol management 
The panel discussion will be moderated by Fleetwood Loustalot, PhD, FNP, FAHA, CAPT, Epidemiologist, CDC Division for Heart Disease and Stroke Prevention and will feature Adrienne Mims, MD, MPH, FAAFP, AGSF, Vice President, Chief Medical Officer at Alliant Health Solutions and Angela M. Reminder: Opportunity for Including Community e-Connect in Year 2

Community e-Connect is a bi-directional linkage between clinical electronic health records (EHRs) and community-based organizations (CBOs). Clinical providers electronically refer patients with diabetes, pre-diabetes, or hypertension to CBOs with programs that can help control these conditions. In turn, CBOs document a patient’s enrollment, their attendance and progress, and electronically send this information back to the clinical provider, where it goes directly into the patient’s EHR.

Six states are currently utilizing their Year 1 funding to implement Community e-Connect, and NACDD is offering the opportunity to additional states for Year 2.  Information is available here. If you are interested in finding out more about Year 2 participation, please contact Susan Svencer,
