State Health Departments Announced for GIS Project
The CDC Division for Heart Disease and Stroke Prevention, NACDD and Children's Environmental Health Initiative (CEHI) at Rice University are pleased to announce the selection of Minnesota and Vermont Health Departments for participation in the 2019 GIS Translation/Dissemination Capacity Building for State Health Departments: Advancing the Translation and Dissemination of Maps for Heart Disease and Stroke Prevention project. Teams from each state and their partners will work with staff from NACDD, CDC, and CEHI in the development and dissemination of translation products for maps related to heart disease and stroke prevention. CDC and NACDD look forward to this exciting work and sharing information on the process and final products developed by each state.

Reminder: Grant Opportunities 
  1. Improve Prediabetes Recognition and Diabetes Prevention Strategies among African American and Hispanic Women The American College of Preventive Medicine (ACPM) will award multi-year grants to three health care organizations to test and evaluate innovative approaches to screen, test, and refer patients to CDC-recognized lifestyle change programs supported by the National Diabetes Prevention Program. Interested health care organizations should complete the online application form no later than March 19, 2019 at 5:00 p.m. ET.

  2. Demonstration Projects for Providers to Reduce Hypertension among Priority Populations The American College of Preventive Medicine (ACPM) has a partnership with the Division for Heart Disease and Stroke Prevention at the Centers for Disease Control and Prevention to increase physician awareness and practices to prevent, detect, and control hypertension among priority populations, and refer patients with hypertension or at high-risk for hypertension to community evidence-based self-measured blood pressure (SMBP) programs with clinical support and other related evidence-based lifestyle change programs. Interested health care organizations should complete the online application form no later than March 30, 2019 at 5:00 p.m. ET.
