Montana’s Crystelle Fogle and Carry Oser in Health Payer Intelligence Value-Based News

Active CVH Council member, Crystelle Fogle, and GIS Network and ECC member, Carrie Oser were quoted in Value-Based News: Strategies for Managing Chronic Conditions in Rural Areas on “Team Up. Pressure Down.” This program worked with community pharmacies to promote medication adherence among patients with hypertension. Read a summary of Montana’s Team Up. EHR-based Surveillance Learning Community

This month, we will be providing practical tips to help with building support for EHR-based surveillance for Chronic Disease, including sharing best practices, performing stakeholder analysis, nurturing excitement and willingness to transform and share data, and much more. Deirdre Browner, with the County of San Diego Health and Human Services Agency, will also share lessons learned from the field. Additionally, we will explore critical success factors to implementing surveillance systems for Chronic Disease Surveillance.
