Sarah Brokaw


Sarah Brokaw, MPH


Sarah Brokaw is a Public Health Consultant in the Diabetes Practice Area at NACDD. Her experience has focused on health behavior change, health education, and implementation of evidence-based practices to create healthy communities. She is passionate about the National Diabetes Prevention Program and the positive changes it has on people.

In her previous role, Sarah was the Manager of the Diabetes Program and the Asthma Control Program for the Montana Department of Public Health and Human Services. She also has conducted patient education and risk communication projects with the Health Behavior and Health Education Genetics Research Group at the University of Michigan.

Sarah earned a BS from St. Lawrence University and an MPH from the University of Michigan School of Public Health.

Sarah lives in Helena, Mont., with her husband Jeremy, son Henry, and dog Layla. She enjoys traveling, baking, hiking, and skiing.
