NACDD Services Assist Your State to Fully Develop Coordinated Chronic Disease Programs and More

 
As your state encounters various service needs such as preparing to develop your Coordinated Chronic Disease Program and accompanying Strategic Plan, Training and Communications Plans, NACDD’s robust catalog of resources can help.  

As your professional association, NACDD has developed a catalog of services to meet the ever changing needs of state chronic disease programs and practitioners.

Most NACDD services are strategically developed around three general areas:  State and Community Technical Assistance, Education and Training Programs, and Programs for Assessment, Planning and Demonstrating Effectiveness.  In each ofthese structured offerings, NACDD provides the option to customize its resources to meet the individual needs of each state or community. 

NACDD services that address all four of the "domains" that focus on developing a coordinated chronic disease program are able to be included in a multi–component approach.  These domains are: Epi/Surveillance, Environmental Approaches, Health Care System Interventions, and Community Programs Linked to Clinical Services.
