Cardiovascular Health

Cardiovascular health refers to the health of the heart and blood vessels. Cardiovascular disease is a group of diseases of the heart and blood vessels, including coronary heart disease, stroke, heart failure, heart arrhythmias, and heart valve problems. There are several risk factors that lead to the development of cardiovascular disease, including high blood pressure, high blood cholesterol, tobacco use, and diabetes.

NACDD Action on Cardiovascular Health

NACDD collaborates with State Health Department programs and other partners working in heart disease and stroke prevention and in healthcare systems to conduct innovative educational and capacity-building opportunities related to CDC-RFAs DP18-1815 and DP18-1817 Category B funding and Million Hearts®2022. NACDD manages a Cardiovascular Health Network, a national group of State Health Department program managers, staff, epidemiologists, and evaluators working to improve diagnosis and management of high blood pressure and high blood cholesterol in their states.

This CVH Network supports health departments in implementing evidence-based, population health improvement strategies in healthcare and community systems, work that is primarily associated with CDC’s Division for Heart Disease and Stroke Prevention-funded cooperative agreements. All states are funded through these cooperative agreements and participate in the CVH Network. The NACDD CVH Network and its CVH Council represent the interests of State Health Departments.

Technical Assistance

Learning Communities

In coordination with CDC’s Division for Heart Disease and Stroke Prevention, NACDD worked with seven state teams on a learning program to accelerate team-based care to manage high blood pressure entitled Accelerating Team-Based Care through the Use of Collaborative Practice Agreements and the Pharmacists' Patient Care Process to Manage High Blood Pressure. Teams from the program now serve as facilitators-in-training and host workshops and trainings within their states to spread successful strategies and share resources developed by CDC and pharmacy partners.

Fireside Chats

NACDD, in coordination with CDC, has developed an innovative format for live webcam presentations, called Fireside Chats, that allow for increased interaction between content experts and State Health Departments working on emerging issues in cardiovascular health. Unlike a webinar, where the bulk of the time is devoted to a speaker presentation based on prepared slides, Fireside Chats provide a more informal opportunity for a brief expert presentation and a specific state example, with the majority of the time being allocated to interaction among state, CDC, and NACDD representatives.

Virtual Roundtables

The CVH Team hosts virtual roundtables to provide states with an opportunity to discuss topics covered during Fireside Chats in a more informal format. Virtual roundtable sessions are not recorded, but summaries of the discussion can be found here.


The CVH Team also hosts webinars using the more familiar format devoting the bulk of the event to speaker presentations on a specific topic with a Q&A period at the end.


Self-Measured Blood Pressure (SMBP)

CDC and NACDD commissioned the Department of Health Policy and Management at the George Washington University Milken Institute School of Public Health to develop A National Analysis of Self Measured Blood Pressure Monitoring report to support efforts to scale and spread the use of SMBP. Issue Briefs

NACDD summarizes topics addressed during the fireside chats and virtual roundtables in Issue Briefs designed to capture key themes of the fireside chats and to provide examples of State Health Department activities related to the topics discussed. Issue Briefs currently available include:

  • Data-Driven Systems Change and Improvement in Hypertension Management
  • Addressing Health Disparities in Hypertension Control
  • Team-Based Approaches to Controlling Hypertension
  • Undiagnosed Hypertension

Success Stories

NACDD assists State Health Departments in promoting successful programs to improve the health of communities and to reduce the burden of chronic disease across the U.S. through development of success stories. NACDD distributes a weekly communication, Off the Cuff, that includes information for the CVH Network on: pertinent webinars and calls; national partners’ announcements and resources; and tips from states on program, epidemiology, surveillance, and evaluation topics.

Off the Cuff reaches more than 500 people each issue, including federal, national, state, territorial, and local public health professionals.

Public Health Data Systems

NACDD is working with states to promote the adoption of innovative surveillance approaches to spread and evaluate evidence-based strategies to reduce risks, complications, and barriers to the prevention and control of cardiovascular disease.

In partnership with CDC and the Children's Environmental Health Initiative at Rice University, NACDD coordinates virtual and in-person training opportunities in Geographic Information Systems (GIS). GIS enables users to visualize and analyze spatial information using maps to provide insights into patterns and relationships, helping state and local health departments in strategic decision-making.

NACDD provides technical support for states implementing Community e-Connect, a bi-directional electronic linkage between clinical Electronic Medical Records and community-based organizations. Community e-Connect creates a collaboration between clinical organizations and community partners who provide evidence-based programs to improve individual patient health as well as population health, overall.

CDC has funded NACDD to develop a pilot project to implement Multi-state EHR-based Network for Disease Surveillance (MENDS) in three State Health Departments: North Dakota, Virginia, and Washington, and their data partners.

National Partners

NACDD continues to work with national partners, including Million Hearts® and the American Heart Association, to support hypertension and cholesterol management.

Million Hearts®

  • NACDD has been a proud national partner of Million Hearts® since 2012 and works with CDC and many other partners to support the priority areas of Million Hearts®. The Million Hearts® Collaboration, facilitated by the American Heart Association, supports improving cardiovascular health outcomes in a variety of sectors and increases communication and dissemination of key strategies, tools, and evidence-based resources to accomplish established goals.
  • The Million Hearts® Hospitals and Health Systems designation was developed to encourage and recognize hospitals that adopt evidence-based, systematic approaches known to improve quality of care and cardiovascular health.
  • NACDD is part of the Self-Measured Blood Pressure (SMBP) Forum facilitated by CDC and is supporting CDC efforts to conduct a national coverage and reimbursement analysis focused on SMBP.
