What's Working in Chronic Disease Prevention and Control

NACDD supports state health departments and works with CDC and other partners to promote programs to improve the health of communities and reduce the burden of chronic disease across the U.S. The resulting success stories demonstrate to policymakers programs that work, show decision-makers that funds are well spent, and serve as program examples for states.
